Headaches your entire life and have become quite knowledgeable in the field of rebound and chronic daily headache. You make a good point that there are patients who develop a pattern of chronic daily headache despite not overusing caffeine or analgesics. In fact, even patients who do have rebound headache generally require a daily preventive medication since their headaches do not usually completely resolve when they get off of the offending agent s ; . I not know your entire medical history, whether you were on appropriate doses of these preventive medications or whether you were off any potential rebound-causing medications while you were using the preventive agents. However, especially in headache specialty centers, we do see patients who have failed numerous therapies in the past. At this point, they may benefit from experimental approaches that are not "standard of care." Rather than subject you to Imitrsx on a daily basis, since we are still not certain of long-term adverse effects even if it did give you complete relief which it does not ; , I would see a healthcare provider who specializes in headache. Common Brand Name s ; : Imitre Uses This medication is used to treat migraine headache attacks once they occur. It is not effective in preventing migraines. This medication is not to be used for other types of headaches. How to Take this Medication Review the information sheet in the product package for full details of use. The usual dose is one spray into one nostril as the headache occurs. If the headache returns, a second spray may be used anytime after at least two hours pass from the first dose as directed. Use of more than 40 mg in one day is not recommended. Follow your MD and pharmacist directions exactly. Side Effects This medication may initially cause flushing, dizziness, weakness, nausea, drowsiness, stiffness, or feelings of tingling, heat or cold and fatigue. Burning, numbness or soreness of the nose infrequently occur. If these persist or worsen, notify your doctor. Unlikely but report promptly if you develop: tightness or pressure in any part of the body, chest pain. In the unlikely event you have an allergic reaction to this drug, seek immediate medical attention. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice other effects not listed above, contact your doctor or pharmacist. Precautions Before using this drug, tell your doctor your medical history, including: allergies especially drug allergies ; , heart disease or family history ; , high blood pressure, high cholesterol, strokes, diabetes, tobacco smoking, kidney disease, liver disease. There have been rare reports of those with sulfa drug allergies having allergic reactions to sumatriptan. Consult your doctor or pharmacist. Since this medication may cause drowsiness, use caution operating machinery or engaging in activities requiring alertness. Limit alcohol use, which can aggravate drug side effects. The manufacturer does not recommend use of sumatriptan in the elderly, because they may be more sensitive to its side effects. This medication should be used only when clearly needed during pregnancy. Discuss the risks and benefits with your doctor. This drug is excreted into breast milk. Consult your doctor before breast-feeding.

Fatigue Fatigue is the principal side effect of interferon and ribavirin therapy in each trial listed in Table 1. This is partly a direct side effect of interferon-alpha, with its stimulation of the immune response giving the release of cytokines and other factors that cause fatigue, and is partly related to the anemia caused by ribavirin. Ribavirin is directly toxic to red blood cells, causing them to break down, giving a median decline in hemoglobin of approximately 3 g dL, 3, 4 ; which will cause most patients to feel fatigued. Fatigue also can clearly be multifactorial, with insomnia, dehydration, and anorexia contributing in individual patients. Management 1. Encourage patients to get plenty of sleep, keep well-hydrated, and eat well-balanced meals to maintain their weight if possible. 2. Acetaminophen Tylenol ; 2 gm per day orally in divided doses, also can be very helpful in combating fatigue, particularly with 325-1, 000 mg as a premedication prior to an interferon dose if the fatigue is worse 1-2 days after an interferon injection. Headache Headache is very common and can be a direct side effect of interferon. It also can result from the development of anemia. Management 1. Steps outlined for fatigue. 2. Nonsteroidal anti-inflammatory drugs NSAIDs ; such as aspirin and ibuprofen also may be helpful, but generally should not be used in patients with significant liver scarring or cirrhosis, who are at risk for gastrointestinal bleeding or renal failure due to NSAIDs. 3. Antimigraine medications such as Imitrwx may be helpful for patients who have a history of migraines.

THE LANGUAGE USED TO DISCUSS THE PROBLEM In this report we have chosen to use the equivalent terms used for humans to describe what is happening to wombats. We do this because we believe that an impediment to people understanding the issues is often the confusion in the language used. We note that when people write about mange in humans they use the word scabies, this makes other people think there is some difference between mange and scabies. That is wrong. Mange and scabies are different names for an infestation by the same creature. We note that both scabies and mange are often called "infections". That is wrong. Scabies-mange is a parasitic infestation. The mite that causes it is a parasite. An infestation can lead to infection, but an infection is caused by bacteria entering open wounds. Where we quote someone who makes this error you will see sic ; written after the word infection. An additional problem occurs when authors use the word "infected" to refer to their behavior in placing mites on wombats. That is wrong. They have infested wombats with S abiei mites. We have chosen to use the word "INFESTED" to remind readers we are not discussing a disease, we are referring to a parasitic infestation. Parasites feed off another animal, and that animal is called the host. In the context of this report, the wombat is the host to the mite. The adult mite has eight legs, so is classed in with spiders rather than insects that have only six legs. Spiders are arachnids. This is where the word acaracide, a product which kills cide ; arachnids comes from. We also use the terms eradicate or fix rather than cure to again remind readers this is a parasitic infestation not a disease. Where we believe terms can be misinterpreted or prejudice an appropriate response we use either both the scientific and common understanding term together or the later alone. So we will use the word epidemic rather than epizootic to refer to unusual increases in infestations and endemic rather than enzootic to refer to usual infestations. An epidemic or an epizootic are abnormal infestations while endemic or enzootic refers to something that is normal to the population. There is a mite found on healthy wombats Acaroptes ; that causes no problem to them, these mites are enzootic or endemic mites while the scabies S abiei ; mite is abnormal and causes problems and is epidemic or epizootic. We use the term naturally raised wombat to refer to a wombat that was reared in the wild by another wombat. We use the term hand reared to refer to wombats raised by humans. We use the term free wombat to describe one that lives naturally, uncaged and unfenced and captive wombat to refer to one being held in an unnatural, human controlled environment such as a zoo, laboratory or other confined environment. There can then be four descriptive terms, free hand reared, captive hand reared, free naturally reared and captive naturally reared. These terms become important when we look at studies done on wombats. While there isn't a great deal around, much of what has been done can be.

Postmarketing Experience With Sumatriptan: Serious cardiovascular events, some resulting in death, have been reported in association with the use of IMITREX Injection or IMITREX Tablets. The uncontrolled nature of postmarketing surveillance, however, makes it impossible to determine definitively the proportion of the reported cases that were actually caused by sumatriptan or to reliably assess causation in individual cases. Phase I Data Presented at the 50th Annual Scientific Meeting of the American Headache Society Demonstrate That NP101 Delivers Sumatriptan in a Rapid, Predictable, and Consistent Manner CONSHOHOCKEN, Pa.-- BUSINESS WIRE ; --NuPathe Inc., a privately held specialty pharmaceutical company developing innovative products for the treatment of neurological and psychiatric diseases, announced positive Phase I results for NP101, a novel drug-device patch for acute migraine in clinical development. NP101 combines NuPathe's SmartReliefTM proprietary iontophoretic transdermal technology with sumatriptan, the most widely prescribed treatment for acute migraine in the United States and the active ingredient in Imitrex. The study was a Phase I, single center, single-dose, open label, five-way crossover study to compare the pharmacokinetics of two NP101 patch formulations with three FDA approved formulations of Imitrex 20 mg nasal spray, 100 mg oral tablet, 6 mg subcutaneous injection ; in 23 healthy volunteers. Tolerability and safety were assessed for all formulations and skin evaluations were performed for subjects after administration of NP101. Results of the study were presented June 28, 2008 at the American Headache Society meeting in Boston, MA. The plasma concentrations for NP101 were more consistent among subjects compared to either the 20 mg nasal spray or 100 mg oral tablet formulations. "These findings support the hypothesis that parenteral administration of sumatriptan, either through subcutaneous injection or transdermal delivery, provides a more predictable method of drug delivery by bypassing absorption through the gastrointestinal system, " said Mark Pierce, M.D., Ph.D., chief scientific officer at NuPathe who presented the results in Boston.
